Job Summary:
The RRT/Regulated Health Professional with Certified Respiratory Educator designation (RHP CRE) is responsible for assessing and delivering clinical services to community-based lung health clients and providing training and support to community-based health care providers for the delivery of best practices and standardized tools and pathways related to COPD and other chronic lung illnesses. The RRT/CRE facilitates the HPE Lung Health Program – a community-based multi-partner initiative aimed to promote best practices and clinical pathways related to the assessment, diagnosis, treatment, rehabilitation, and self-management of individuals with COPD, asthma, and other chronic lung disease living in the Quinte area.
Accountability:
This position is accountable to the BQWCHC Manager of Clinical Regional Programs, collaborating with HPE Lung Health Network.
Responsibilities:
Clinical:
- Provides clinical assessments, spirometry testing, education/ counselling in multiple primary care organizations
- Consults with, educates and makes treatment and therapeutic recommendations to primary care providers and other primary care staff in accordance with best practice guidelines
- Supports the growth and use of pulmonary rehab, peer support education, and client self-management action plans
- Works collaboratively with, and assists to build capacity for evidence-informed practices within interdisciplinary teams among HPE Lung Health Network partners
Program Development:
- Assist with the development and implementation of the HPE Lung Health Network workplan with a goal to delivering best practices for lung health along the care continuum from prevention to diagnosis, to self-management, to rehabilitation, to end-of life care, related to COPD
- Liaise with community members, network partners, specialists, and potential partners
- Assists with the collection and monitoring of quality and performance measures, and activity stats
Qualifications:
- Registered Respiratory Therapist in good standing with the College of Respiratory Therapists of Ontario or other Regulated Health Professional with Certified Respiratory Educator (CRE) designation in good standing with their respective college
- If RRT, Certified Respiratory Educator status required (or willing to obtain 12 months)
- Recent clinical RRT/RHP-CRE experience, preferably in a community setting
- Experience with multidisciplinary teams
- Experience in program development, implementation, monitoring, and evaluation, preferred
- Competency in spirometry testing in accordance with ATS Standards, preferred
- Excellent communication, time management skills & the ability to maintain flexible schedule
- Valid driver’s licence: use of a reliable insured vehicle and willingness to travel within HPE
- Support the mission and values of BQWCHC and partner agencies within the Network
- Proficient computer skills in MS Office applications (Word, Outlook, PowerPoint)
- Knowledge of related local community partners and resources is preferred
- Experience with data-driven quality improvement practices
- Experience with electronic medical records and collaborative
